Date format

Hello All,

I need help on the date format. I need to display date in this format  "Last Run Date: 05/26/2015 12:04 PM"

Here is the formula I have written

"Last Run Date: " + FormatDate(CurrentDate(); " MM/dd/yyyy hh:mm AM/PM ")

It is displaying as “    Last Run Date: 05/26/2015 02:38 AM5/P5

Any help would be appreciated.
